How We Create the Right Care Plan for Your Family

Phase 1 — Initial Inquiry & Assessment

The moment you contact SLR Home Care, we start listening. On your first call, one of our care specialists gathers the key details — what's going on, who needs support, and what kind of help makes the most sense. We respond within hours, not days. From there, we schedule a no-obligation assessment — either in-home or virtually — so we can truly understand your situation before recommending anything.

Phase 2 — We Build A Personalized Care Plan

After your assessment, we build a fully customized Family Care Plan — designed around your loved one's routines, preferences, personality, and specific needs. The plan is completely transparent. You'll always know what services are being provided, and you'll be notified before any changes are made. Nothing happens without your knowledge and approval.

Phase 3 — Care Assignment & Agreement

Once your care plan is approved, we get to work finding the right PSW for your family — not just someone with the right skills, but someone whose personality, communication style, and values align with your loved one's. We believe the relationship between a PSW and the person they care for matters just as much as the tasks they perform. If for any reason the fit isn't right, we'll make a change — quickly and without hesitation.

Step 4 — SLR Care & Ongoing Support

Your PSW keeps detailed care notes after every visit. You'll receive clear, itemized billing with no surprises. Our team conducts regular check-ins to ensure care quality is consistently high — and we remain available to you virtually around the clock. Your peace of mind doesn't end after day one. We're in this with you for the long haul.
